Ray is the Senior Consultant for HPN Worldwide. He works with organizations, helping them persuade participants of
health benefit plans to become more engaged, knowledgeable and assertive with respect to health improvement and
their use of healthcare. In his presentation, Ray discusses the emergence of MultiShare, Community Health Plans and
especially highlights the phenomenal outcomes of one such organization that has been active in Muskegon, Michigan
since 1999. This is a community plan that has allowed member organizations (all small businesses) to avoid the
typical health plan cost increases over an elongated period of time.

Medical Home - An Investment in Primary Care
Martin Manning, Chief Executive Advocate Physician Partners
In his current role, Marty is responsible for all contracting activities for eight Advocate Physician Hospital Organizations,
a group currently serving some 230,000 capitated lives. In his presentation, Manning discusses the idea of every patient
having a "Medical Home" in the form of a primary care physician that practices evidence based medicine. Listen especially
as he explains Advocate's advanced "way finding" that helps direct patients through the healthcare system and to the most
quality based and cost effective care.

Impacting the Delivery of Care
Mark Shields, MD, VP Medical Management Advocate Physician Partners
In his current role, Mark oversees all clinical functions related to 3,400 physicians aligned with the nine Advocate
hospitals. In his presentation, Mark outlines Advocates's current strides in its pursuit of Clinical Integration (CI).
His description as how CI provides the infrastructure for Medical Home, and is further a framework for the Accountable
Care Organizations, previously described by the current Health Reform Law, gives great insight as to how the provider
of the future will function.

David has been in the healthcare industry for 35 years and is a recognized innovator of managed care services and strategies
focused on more effective provider/member partnerships. In his presentation, David explains why, under the new Health Care
Reform, self funding of employee benefit plans remains one of the only alternatives allowing enployers to "customize" benefits
to their specific employee populations and health status. Even as we wait on all the "interpretations" which will be determined
by forthcomming regulations, David outlines why self funding is more viable in the "post" health care reform era.
